Job summary

Pioneer Human Services seeks a Health Coordinator (FT/DAY) in Washington to coordinate and monitor client health services within residential treatment facilities. You will liaise with clients, nursing staff, providers, and program staff to ensure continuity of care and accurate health documentation.

In this role you will assist with intake, schedule appointments (including telehealth), monitor medications in MAR/eMAR, and provide direct client support in a structured environment while responding

Qualifications

• High School Diploma or equivalent required.
• One year of caregiving, human services, behavioral health, or related experience.
• Must obtain Agency Affiliated Counselor registration within 30 days of hire; CPR/First Aid within 90 days.
• Strong written and verbal communication, and attention to detail.
• Proficient computer skills and ability to learn new systems.

Responsibilities

• Coordinate and monitor client physical and behavioral health services in residential facilities.
• Support intake, telehealth scheduling, and medication monitoring in MAR/eMAR.
• Provide direct client support in a structured environment and respond to urgent health concerns.
• Assist with care plans and ensure accurate health documentation in systems.
• Collaborate with nursing staff, providers, and program staff to ensure continuity of care.
