Job summary

Qualifications

• Requires BA/BS degree in a health care related field; field of study preferred.
• Community Health Worker certification to be obtained within one year of employment (MMP).
• Minimum of 1 year of experience working with the program population or related community-based organizations.

Responsibilities

• Manage member's individual service plan within applicable state law and contract.
• Conduct telephonic or face-to-face history and needs assessments to identify and coordinate program needs.
• Coordinate cases with clinical healthcare management and interdisciplinary team for care coordination support.
• Manage non-clinical needs of members with chronic illnesses, co-morbidities, or disabilities.
• Establish short and long-term goals with members, caregivers, and physicians.
