Job summary

Qualifications

• Requires BA/BS degree and a minimum of 1 year of experience working directly with people related to the program population or related community organizations.
• BA/BS degree field preferred; specific education and years/experience may be required by state law and contract.
• Must reside in Buncombe County and travel to worksite as needed.

Responsibilities

• Manage member's individual service plan within state law and contract requirements.
• Conduct telephonic or face-to-face history and needs assessments to identify program needs.
• Identify members with potential clinical needs and coordinate care with clinical teams.
• Manage non-clinical needs of members with chronic illnesses to ensure cost-effective service use.
• Set short and long-term goals with members, caregivers, and physicians.
• Identify members who would benefit from expanded services.
