Job summary

Qualifications

• Requires BA/BS degree and at least 1 year of related experience.
• BA/BS field in health care preferred; state law may affect requirements.
• Travels to worksite and other locations as necessary; Community Health Worker cert within 1 year for MMP.

Responsibilities

• Responsible for overall management of a member's individual service plan per state law and contract.
• Conduct telephonic or face-to-face history and program needs assessments.
• Identify members with potential clinical health care needs and coordinate with clinical teams.
• Manage non-clinical needs of members with chronic illnesses to ensure cost-effective service use.
• Establish short and long-term goals with member, caregivers, family, and physicians.
