Job summary

Arizona Youth Partnership seeks a Health Navigator to provide individualized support and care coordination for participants, focusing on pregnancy, parenting, recovery goals, and access to healthcare and social services.

Qualifications

Responsibilities

Job description

TITLE: Health NavigatorHOURS: 40 hours per week, some evenings, weekends (as needed)SUPERVISED BY: LINKS Program ManagerDUTIES AND RESPONSIBILITIESThe Health Navigator provides individualized support, encouragement, advocacy, care coordination, and navigation services to participants. The Health Navigator assists individuals in identifying their strengths, needs, developing recovery, wellness goals, help in accessing healthcare, treatment, pregnancy and parenting needs, and social services.Provide support, encouragement, advocacy, and guidance to participants throughout their engagement in services.Conduct program intake and complete screening and needs assessments related to physical and mental wellness, substance use history, recovery needs, birthing and parenting needs, and participant preferences.Assist participants in developing individualized birthing plans, recovery and wellness goals, and support plans.Provide care coordination and referral management across treatment, childbirth, post-partum care, recovery, healthcare, and social service systems.Support participants with linkage to medication for opioid use disorder (MOUD), medication-assisted treatment (MAT), behavioral health services, primary healthcare, substance use treatment, recovery services, and other needed supports.Coordinate service needs based on the participant’s pregnancy and parenting plan and assist in identifying supports and resources.Follow up on referrals to ensure participants are successfully connected to needed services and address barriers to accessing care.Maintain regular contact with participants to monitor progress, provide encouragement, and support continuity of care.Collect and document program performance measures and participant outcomes as required.Complete all fund source and AZYP reporting requirements.Assist with monthly, quarterly, and annual reporting of actions and outcomes for all funding streams.Represent AZYP at public events, as directed, to promote prevention efforts.Participate in trainings as required by funder or AZYPOther duties as assigned by supervisorQUALIFICATIONS:High school diploma or GEDInitiating and independent worker. Creative, flexible, collaborator, team-playerExperience working with pregnant and parenting individuals, families, individuals experiencing substance use challenges, or individuals involved with health and social service systems preferred.Ability to maintain confidentiality and professional boundaries.Excellent presentation, oral, written communication skills; knowledge of Word, Excel, Publisher, Power Point a plusREQUIREMENTSFinger printing and Background Check; CPR/First Aid Certification.AZ Driver’s License, reliable transportation, and ability to travel countywide and statewide.Automobile Insurance in the amount of $100,000/$300,000Some evening and weekend hours as needed
