Job summary

Family Health Centers of San Diego is seeking an HIM Clerk to prepare patient charts, manage medical information requests, and support data accuracy in a fast-paced clinic environment.

The role requires strong medical terminology knowledge, experience with EHR and practice management systems, and the ability to work well with clinical staff. Bilingual English/Spanish is preferred, with high school completion and terminology training mandatory.

Qualifications

• High school diploma or GED required.
• Medical Terminology course required or completed within 90 days.
• Medical Assistant Certification preferred.
• RHIA or RHIT Certification preferred.

Responsibilities

• Prepares patient charts for upcoming visits, reviews open orders, incomplete documentation, referrals, and pending results.
• Coordinates release of medical information per guidelines.
• Verifies authorizations, processes requests, and communicates with requestor.
• Provides customer service for department via phone, email, and visitors.
• Reviews medical records for accuracy and completeness; participates in QA/QC and data abstraction.
• Generate billing statements and record all billing activities.
• Ensure HIPAA/privacy compliance for PHI sharing.
• Demonstrate strong medical terminology and documentation skills.
• Integrate data from electronic and paper sources and support HIM workflow.
