Job summary

Cook County Health & Hospitals System is seeking an HIM Coder at John H. Stroger, Jr. Hospital to abstract clinical data, assign ICD-10-CM/PCS, CPT-4, and HCPCS codes, and ensure accurate documentation for reimbursement and compliance.

The role supports inpatient and outpatient coding with a focus on accuracy and efficiency. The position requires a high school diploma or GED, at least one year of coding experience, and relevant AHIMA or AAPC certifications.

Qualifications

• High School Diploma or GED required (must provide at interview).
• 1 year of coding experience within the last two years required.
• Applicants must possess one or more of the following certifications: RHIA, RHIT, CCS, CCS-P, CPC.

Responsibilities

• Identify principal and secondary diagnoses and procedures using ICD-10-CM, CPT-4, and HCPCS Level II with minimal errors.
• Sequence diagnoses and procedures in the appropriate order.
• Abstract data into the clinical abstracting database and maintain accuracy.
• Utilize encoder/abstracting systems to minimize coding errors.
• Maintain productivity standards and ensure 95% or above quality accuracy.
