Responsibilities

• The Health Information Assistant provides support to staff and the HIM Department by assisting with monitoring and maintaining complete Health Records of all persons served.
• Ensures professional contact with all levels of staff and stakeholders, including persons served and families.
• Reports to the Health Information Coordinator and works closely with Nursing/Clinical Directors and department heads.
• Receives consultation, advice and mentorship in technical record matters from the Health Information Coordinator.

Title: Health Information Assistant

Job Duties: The Health Information Assistant provides support to staff and the HIM Department by assisting with monitoring and maintaining complete Health Records of all persons served. Accuracy and thoroughness are important, as they may affect the legal liability of the facility. Ensures professional contact with all levels of staff and stakeholders, including persons served and families. The Health Information Assistant reports to the Health Information Coordinator, works closely with the Director of Nursing, Program Director/Clinical Directors, and other department heads. The Health Information Assistant receives consultation, advice, training, and mentorship in technical record matters from the Health Information Coordinator, in addition to our corporate Health Information Department, as required by program-specific regulations.

Qualifications:

• High school graduate or equivalent
• A minimum of one year of experience working in a health-related field
• Knowledge of medical terminology
• Knowledge of governing regulations

About the Campus: The Crestwood Center San Jose Psychiatric Health Facility (PHF) on our San Jose campus is in close proximity to the mental health support services in Santa Clara County. The San Jose PHF provides a spacious environment, with two living rooms, a welcoming room, visiting space, serenity room, dining room and a beautiful courtyard. The program is dedicated to restoring a sense of hope, self-empowerment and recovery in each of our clients. The San Jose PHF is designed for clients who require stabilization at a time of acute psychiatric crisis. Mental Health Rehabilitation Centers (MHRCs) provide psychosocial rehabilitation programs in secure, residential settings with a focus on brief lengths of stay. The mission of this program is to restore a sense of hope, empowerment, community/natural support inclusion, and realized potential in each client, as well as psychiatric stabilization.

The salary range listed below represents the minimum and maximum base pay per hour at the time of posting. Final salary offered to the candidate selected for the position will be based on factors including but not limited to candidate's skills, experience, licensure, and program acuity.

Pay Range:

$23 - $23.93 USD
