Qualifications

• Master’s degree in social work, psychology or related field with 1 year of experience; OR
• Bachelor’s degree in social work, psychology or related field with 2 years of experience; OR
• Credentialed Alcoholism and Substance Abuse Counselor (CASAC) with 2 years of experience; OR
• Bachelor’s degree with 3 years of experience in health home care management.

Responsibilities

• Coordinate care for health home members with complex medical/psychiatric needs.
• Develop individualized care plans with primary care providers and specialists.
• Ensure timely access to medical, behavioral health, and social services.
• Maintain accurate documentation in electronic case files.
