Qualifications

• High School Diploma or equivalent is required.
• 1-3 years of relevant experience required.
• Bachelor's or Master's degree in a relevant field strongly preferred.
• CASAC credential with two years experience or a Bachelor's degree with three years Health Home experience highly preferred.
• NYS Driver's License strongly preferred.

Responsibilities

• Uses patient-centered motivational interviewing to build rapport and improve health.
• Supports the primary care team by providing panel management to reduce care gaps.
• Liaises between hospitals, PCPs, specialists, and community resources to coordinate care.
• Identifies high-risk and other populations for preventive or chronic care services.
• Tracks patients discharged from inpatient or ER settings.
• Uses team communication to close referrals and follow-ups; conducts outreach in clinics, homes, and hospitals.
