Job summary

Qualifications

Responsibilities

• Establish trust and provide outreach, advocacy, support, and education to develop patient self‑management skills.
• Support a proactive, multidisciplinary team approach to prevention, education, and health promotion.
• Document sessions and progress through appropriate documentation avenues.
• Collaborate with providers and other resources to arrange further care when necessary.
• Consult with members of the health care team to solve problems and seek advice.
• Maintain patient privacy and confidential patient information.
• Participate in outreach and follow‑up phone calls and visits.
• Share questions and concerns with appropriate resources.
• Assist in the development of criteria to measure outcomes and in the analysis of data for quality improvement, interpreting results, identifying limitations and barriers, and recommending alternatives.

Responsibilities

• Ability to establish relationships with key professionals in all areas.
• Valid Driver’s License required with a good driving record.
• Basic Life Support (BLS) may be required based on facility needs.
