Job summary

Qualifications

• 18 months supporting marginalized, high-risk populations.
• Proficient in documentation and communication using computers.
• Personal lived experience connecting with served populations.

Responsibilities

• Conduct intake assessments to determine eligibility for services.
• Assist clients with Medi-Cal program eligibility and resources.
• Identify barriers and connect clients to community services.
• Coordinate care for clients with complex health conditions.
• Engage clients with culturally responsive outreach efforts.
