Job summary

Sea Mar Community Health Centers is seeking a Community Resource Navigator to provide information and assistance on health coverage, public programs, and Sea Mar services. You will educate eligible individuals, assist with applications, and schedule initial medical, dental and care management appointments.

Qualifications

• AA or BA in Social Services or Business Administration with at least 2 years of customer service experience.
• Bilingual: English and Spanish preferred.
• Excellent customer service skills.
• Good organizational and time-management skills.
• Proficient in Microsoft Office programs.
• Ability to read, interpret complex documents, reports and data.

Responsibilities

• Proactively raise awareness of health coverage enrollment by planning outreach activities in the community.
• Develop and distribute informational/promotional materials, and maintain referral relationships with local organizations.
• Routinely work uninsured lists to advance enrollment and inform about options.
• Assist consumers with online public health coverage and program applications.
• Serve as a service navigator to establish Sea Mar as the medical home for consumers.
• Help high-risk patients schedule a care manager appointment and remove barriers to care.
• Maintain daily activity records and submit progress reports in a timely manner.
• Identify uninsured barriers and advocate for access to care.
• Attend mandatory trainings and stay current with policy changes.
• Perform other duties as assigned while adhering to policies.
