Position Summary:

Full‑time Community Resource Navigator position available for our Managed Care Department in Mount Vernon, WA. The role involves providing information and assistance to individuals seeking health coverage, health care and other local resources that aid them in maintaining a healthy lifestyle. Responsibilities include educating eligible individuals on how to navigate health insurance options, public programs, and Sea Mar services; assisting with application processes; aiding newly assigned and high‑risk patients in scheduling their initial appointment for medical, dental and care management services; and participating in case‑management and intervention strategies that remove barriers to care. The position also requires meeting all Washington State and Sea Mar testing and certification requirements.

Duties and Responsibilities:
  • Pro‑actively aid in increasing public awareness regarding health coverage enrollment opportunities by planning and conducting outreach activities within the community, and participating at both Sea Mar sponsored and non‑Sea Mar sponsored events.
  • Assist in the development and distribution of informational/promotional materials, and establish and maintain referral relationships with local organizations, agencies, institutions and businesses as directed by the Managed Care Department.
  • Work uninsured lists from Sea Mar dental, behavioral health and ancillary service sites to advance enrollment of eligible individuals onto health coverage and inform non‑eligible individuals about health care options.
  • Assist consumers with online public health coverage and program applications, providing the necessary information for consumers to navigate health plan options and make well‑informed decisions.
  • Serve as a service navigator for consumers wishing to establish Sea Mar as their medical home, by providing information regarding available services and consumer rights and responsibilities.
  • Be responsible for assisting high‑risk patients in scheduling a care manager appointment and facilitating processes to ensure they have established care. Incorporate a case‑management model including home visits if necessary to assist patients who fail to keep their initial medical visit by removing impediments and linking them to necessary resources.
  • Maintain daily activity records, submit progress reports in a timely manner, and assist in identifying areas for quality improvement and efficiency within the Managed Care Department.
  • Identify, monitor and track uninsured individuals and advocate on their behalf regarding barriers to coverage and access of health care services.
  • Perform all duties in a professional manner, participate in all mandatory state and corporate trainings and meet all competency testing requirements regarding health coverage policy changes impacting consumers and processes.
  • Perform any and all other duties as may be assigned while adhering to all corporate and departmental policies and procedures.
Qualifications and/or Requirements:
  • Must possess good organizational and time‑management skills.
  • Demonstrate good judgment and have excellent problem‑solving abilities.
  • Maintain a professional demeanor at all times and ensure confidentiality.
  • Must be proficient at public speaking.
  • Must have good spelling, grammar and written communication; legible handwriting required.
  • Must be able to read and interpret complex documents, reports and data.
  • Must be able to work evenings and weekends as needed.
  • Must be self‑motivated, have excellent work ethic and be able to work independently.
  • Proficient in Microsoft Word and Excel.
  • Marketing and sales experience considered a plus.
  • Bilingual in English/Spanish required.
Education and Experience:
  • AA or BA in Social Services or Business Administration preferred.
  • At least 2 years of customer service experience required; preferably in a health care environment. Additional work experience may be substituted for some education.
  • Position must obtain Basic Life Support (BLS) CPR within 90 days of hire and maintain current BLS CPR throughout employment.
