Job summary

The YMCA is seeking a Community Health Coach in Ruther Glen, VA to facilitate chronic disease health programs, deliver biometric screenings, and provide health coaching. The role supports YMCA initiatives and requires strong facilitation skills and adherence to program guidelines.

You will help recruit participants, maintain confidentiality, and contribute to a welcoming, supportive environment aligned with YMCA values and HIPAA policies.

Qualifications

• College degree in Public Health, Health Education, Exercise Science or related field preferred.
• At least 18 years of age.
• Must comply with organizational policies.
• Experience in the implementation and facilitation of health and wellness or public health programs required.
• Must possess health coaching skills and Listen First/Motivational Interviewing skills.
• Must have access to reliable transportation to travel to multiple off-site class locations.
• Proficient in computer use and Microsoft Office suite.

Responsibilities

• Display empathy towards health seekers with chronic disease.
• Provide motivation, support and guidance to program participants in healthy behavior changes.
• Follow strict program delivery guidelines with consistency.
• Utilize Listen First/Motivational Interviewing concepts in facilitation.
• Ensure participant commitment and retention while achieving quality outcomes.
• Prepare before each session and follow up with participants between sessions.
• Maintain accurate attendance records and program data.
