Job description

We’re partnering with a digital health company to hire remote, full-time Health Coaches focused on supporting patients through remote patient monitoring for chronic condition management.

This is a great opportunity for newer grads (1–2 years of experience) who are passionate about helping people manage conditions like diabetes, hypertension, and cardiovascular disease.

What You’ll Do:

• Lead virtual 1:1 coaching sessions with patients managing chronic conditions
• Provide ongoing guidance and support using remote monitoring tools
• Educate patients on disease management, medication adherence, and preventive care
• Build rapport while helping patients recognize symptoms and take charge of their health

Compensation & Benefits:

• $20–$22/hour
