Remote Position - Must Reside Near Orlando, FL Area

Job Responsibilities:

The Health Coach will be a vital part of the Heart Failure Program, working closely with the clinical team to provide patient education, guidance, and satisfaction, ensuring patient-centered care.

Responsibilities include, but are not limited to:

1. Providing education on chronic disease management, especially heart failure diagnosis and management.
2. Delivering motivational and educational content on heart health, diet, exercise, and medication adherence.
3. Setting and monitoring short- and long-term goals in collaboration with patients.
4. Overseeing educational components, documentation, and patient progress.
5. Encouraging patient engagement, advocacy, and self-development.
6. Following up on patient communications promptly.
7. Ensuring an excellent patient experience.
8. Building rapport and maintaining long-term relationships with patients and caregivers.
9. Collaborating with the clinical team to optimize patient outcomes, escalate concerns, and share care plans.
10. Participating in interdisciplinary team meetings.
11. Performing additional duties as assigned.

Qualifications and Skills:

1. Health coach certification from an accredited program (CAAHEP or ABHES); a bachelor’s in health sciences is preferred.
2. Experience in medical or healthcare settings; cardiology coaching experience is a plus.
3. Experience with telehealth is advantageous.
4. Strong interpersonal and communication skills.
5. Excellent customer service orientation.
6. Proficiency with electronic health records (e.g., Athena) and Microsoft Office.
7. Bi-lingual in English and Spanish is highly preferred.
8. Must have a private, dedicated remote workspace with reliable internet.

Additional Details:

This is a full-time position offering a salary of $25-30/hour, based on experience, with benefits including 401(k), health, dental, vision, disability, life insurance, and paid time off. The role is remote but requires residence near Orlando, FL. The schedule is Monday to Friday.
