Job summary

Fives DyAG Corp. in Baltimore, MD is seeking a Health Care Occupational & Case Manager for a 12+ month contract with hybrid work.

The role focuses on assessing medical, psychological, and social needs, developing care plans, coordinating services, and advocating for patients within the healthcare system. Qualified candidates have a nursing or related background with case management experience, current BSL certification, and an RN degree (BSN/MSN preferred).

Qualifications

• Background in nursing, social work, or related field with case management experience.
• Strong communication, organizational, and problem‑solving skills.
• Current BSL (Basic Life Support) certification.
• RN degree (BSN or MSN) preferred.

Responsibilities

• Assess patients' medical, psychological, and social needs.
• Develop individualized care plans in collaboration with healthcare providers and patients.
• Coordinate healthcare services, including medical treatments, therapies, and consultations.
• Monitor patients' progress and adjust care plans as needed.
• Provide education and support to patients and their families.
• Advocate for patients' needs and rights within the healthcare system.
• Communicate with healthcare providers, insurance companies, and other stakeholders to ensure coordinated care.
• Document patient assessments, care plans, and interactions.
• Keep abreast of healthcare regulations, policies, and best practices related to case management.
• Schedule: 7am-3:30pm; report to location on Thursdays; on-call weekly rotation every 4 weeks; on-call 24 hours for work-related incidents.

Key Responsibilities

• Assessing patients' medical, psychological, and social needs.
• Developing individualized care plans in collaboration with healthcare providers and patients.
• Coordinating healthcare services, including medical treatments, therapies, and consultations.
• Monitoring patients' progress and adjusting care plans as needed.
• Providing education and support to patients and their families.

• Evaluating the effectiveness of care plans and making recommendations for improvement.
• Documenting patient assessments, care plans, and interactions.
• Keeping abreast of healthcare regulations, policies, and best practices related to case management.
• Health Care Occupational and Case Managers should have a background in nursing, social work, or a related field, along with experience in case management.
• They should have strong communication, organizational, and problem‑solving skills.
• A compassionate and empathetic approach to patient care is essential.
• Depending on the specific role, certification as a Case Manager (CCM) or in a related field may be required or preferred.
• Schedule: 7am-3:30pm.
• Will report to location on Thursdays or as needed.
• On-call weekly rotation schedules every 4 weeks.
• Will be on call 24-hours for work‑related injury, illness, or exposure reporting.

Key Requirements and Technology Experience

• Strong customer service, assessment & adaptability skills
• Critical care experience, occupational health, or case management experience
• Current BSL (Basic Life Support) Certification.
• Registered Nurse Degree: Bachelor of Science in Nursing or Master of Science Degree preferred
