JobDiva, Inc. is seeking a Case Manager RN for an onsite role in Tallahassee, FL. This fulltime opportunity focuses on comprehensive patient assessments, care planning, and collaboration to optimize discharge planning and resource use.

The ideal candidate holds ADN or Nursing Diploma, Florida RN license (compact acceptable with 90-day activation), and a minimum of 3 years in acute care case management, including Med/Surg or ICU settings. Evening/weekend shifts are included.

Qualifications

• Minimum 3 years of recent Case Manager experience in an acute care setting.
• ADN or Nursing Diploma; Florida RN license required (compact acceptable; activate within 90 days).
• 3+ years of total acute care experience; Med/Surg/Telemetry/Neuro/ICU/PCU/ED may be considered.
• Onsite in Tallahassee, FL; fulltime position.

Responsibilities

• Conduct comprehensive assessments of patients' medical, psychosocial, and discharge needs.
• Develop, coordinate, and document individualized case management plans of care.
• Collaborate with interdisciplinary teams to manage patient care, level of care, and resource utilization.
• Coordinate discharge planning and identify barriers to patient throughput.
• Facilitate referrals to post-acute services and payer case management programs.
• Maintain accurate documentation of interventions and recommendations.

Pay Range:

• Minimum: $31.94/hour
• Mid: $39.93/hour (10+ years of experience)
• Maximum: $43.92/hour (15+ years of experience)

Shift Differentials:

• Evening: +$2.50/hour
• Weekend: +$2.00/hour
• Differentials stack; evening weekend shifts receive an additional $4.50/hour.

Key Responsibilities:

• Days: 8:00 AM – 4:30 PM, with 1 weekend every 4 weeks
• Evenings: 12:00 PM – 8:30 PM with rotating weekends
• Alternative Evening Schedule: 11:00 AM – 7:30 PM, Monday–Friday; weekday-only roles may rotate weekends as needed
• Conduct comprehensive assessments of patients' medical, psychosocial, and discharge needs.
• Develop, coordinate, and document individualized case management plans of care.
• Collaborate with interdisciplinary teams to manage patient care, level of care, and resource utilization.
• Conduct interdisciplinary team meetings and coordinate appropriate interventions.
• Evaluate admissions and continued stays for medical necessity using approved criteria.
• Assess observation patients for appropriateness of observation status.
• Perform utilization management reviews and communicate with third-party payors.
• Coordinate discharge planning and identify barriers to patient throughput.
• Facilitate appropriate referrals to post-acute services, community resources, and payer case management programs.
• Serve as a liaison between physicians, patients/families, hospital staff, payors, and outside agencies.
• Monitor quality, compliance, patient safety, and regulatory requirements.
• Track and trend barriers and variances to care and recommend process improvements.
• Identify high-risk patients and coordinate appropriate interventions and referrals.
• Support patient education, assistance programs, and community-based resources.
• Promote cost-effective care without compromising patient safety or quality.
• Maintain accurate documentation of case management interventions and recommendations.
• Participate in performance improvement initiatives and mandatory education.
• Report patient safety concerns and significant medical errors according to established procedures.
• Perform other duties as assigned.

Key Requirements and Technology Experience:

• Minimum 3 years of recent Case Manager experience in an acute care setting, with experience within the last year.
• Candidates with 3 years of current experience in Med/Surg, Telemetry, Neuro, ICU, PCU, or ED may also be considered.
• Candidates with Case Management experience in Home Health or Insurance may be considered if they have:
• 3 years of total acute care experience, and
• At least 1 year of acute care experience within the last 5 years.
• Associate Degree in Nursing (ADN) or Nursing Diploma required.
• Current Florida RN license or appropriate compact RN license required.
• If holding a compact license, an active Florida RN license must be obtained within 90 days of hire.
• Advanced Practice Registered Nurse (APRN) license may be accepted if current and compliant.
