Job summary

Northern Montana Hospital is seeking a Health Care Coordinator to assist in managing patients, especially those with chronic disease, and to promote transition of care. The role emphasizes effective communication with patients, families, and other health providers to improve outcomes.

The position requires a health education/nursing/nutrition degree or a related certification, HBS training current or within 3 months, and a valid driver's license.

Qualifications

  • Degree in health education, nursing, nutrition, or certification as Registered Medical Assistant or three years clinical experience in a primary care setting.
  • Up-to-date training and certification in Healthcare Provider Basic Life Support required; must obtain within 3 months if not current at hire.
  • Certification through the National Society of Health Coaches can be pursued; salary increase upon certification.
  • Valid driver's license with acceptable driving record for insurance purposes.

Responsibilities

  • Perform population health, care management, and patient-self management support and clinical documentation.
  • Provide support for patient and family centered continuity of care services during transition of care.
  • Assist with linking patients and families to community resources.
  • Work collaboratively to improve quality of patient care and facilitate efficient use of resources.
  • Advocate for individuals' health care needs and help minimize fragmentation of health delivery systems.

The not-for-profit, 25-bed Critical Access Hospital features a 24-hour physician-staffed emergency department, with state-of-the-art diagnostic imaging and electronic medical record services. Our large, quiet labor/birthing rooms are private and located away from other patient care areas. The private patient rooms on the 5th floor Medical/Surgical Department boast views from every window. Administration, staff, and medical providers are committed to delivering high quality, comprehensive health care services to the people of the Hi-Line communities of Montana.

About the role

The Health Care Coordinators primary responsibility is to assist in the overall management of Northern Montana Hospital (NMH) patients, particularly those with chronic disease. Demonstrates innovation and an acceptance of new ideas and concepts. Shows the ability to resolve conflicts with clients and overcome their objections about their health and behaviors. Possesses above average optimism and the ability to motivate clients to make vital changes in their lives. Exhibits effective communication skills for working with clients, their families and their other healthcare providers. Demonstrates a commitment to clients and a willingness to go above and beyond normal responsibilities to provide the best care possible. Shows self-confidence and the ability to express views that may not be popular or shared by clients. Perform population health, care management, and patient-self management support and clinical documentation functions. Provide support for patient and family centered continuity of care services for patients undergoing transition of care. Assist with providing patient and families with optimal linkage to community resources. Work collaboratively to improve quality of patient care through facilitating the efficient use of resources. Acts as an advocate for individual's health care needs and assists in minimizing the fragmentation of health delivery systems.

Qualifications

Degree in health education, nursing, nutrition, or certification as Registered Medical Assistant or three (3) years clinical experience in a primary care setting, or equivalent education or experience

Will have up to date training and certification in Healthcare Provider Basic Life Support issued by the American Heart Association, American Red Cross or American Health & Safety Institute. Must obtain within 3 months if not current at hire.

Ability to obtain certification through the National Society of Health Coaches. Salary increase upon certification.

Valid driver's license with acceptable driving record for insurance purposes.

Salary is commensurate with up to 10 years of experience.
