Job summary

Americare Senior Living’s Centennial Pointe Memory Care in Springfield, IL is seeking an LPN to lead the clinical team as Health and Wellness Director. You’ll oversee all nursing services, supervise staff, and ensure care plans meet regulatory standards in a resident-centered environment.

You’ll collaborate across departments, guide orientation and training, manage assessments and documentation, and uphold Americare’s mission of compassionate, high-quality senior care.

Qualifications

• Current state LPN license.
• Experience leading nursing teams in senior living or long-term care preferred.
• Strong clinical knowledge and leadership abilities.

Responsibilities

• Oversee the clinical supervision and administration of all nursing services.
• Implement and maintain nursing objectives, standards, policies, and procedures.
• Provide clinical guidance to nursing staff including RNs, LPNs, CNAs, and care teams.
• Coordinate orientation and training for staff across departments.
• Conduct regular resident assessments and maintain up-to-date documentation.
• Ensure compliance with state, federal, and Americare regulations.
• Manage medication systems to meet regulatory guidelines and resident needs.

Your Role as the Health and Wellness Director

As theHealth and Wellness Director, you’ll lead the clinical team to ensure exceptional care for our residents. You’ll oversee all nursing services, providing supervision and guidance while maintaining compliance with policies, regulations, and Americare’s high standards. Collaborating with other departments, you’ll manage assessments, care plans, and staff development, ensuring smooth operations. Your leadership will play a key role in fostering a safe, home-like environment while supporting the well‑being of both residents and team members.

This role may also be known as:

LPN Director of Nursing, Assisted Living Nurse Manager, Memory Care Nurse Leader, Clinical Nurse Supervisor, or Wellness Director.

What You’ll Do

• Oversee the clinical supervision and administration of all nursing services.
• Implement and maintain nursing objectives, standards, policies, and procedures, revising them as necessary.
• Provide clinical guidance to nursing staff, including RNs, LPNs, CNAs, and other care team members.
• Collaborate with community administrators to ensure all staff receive necessary orientation and training.
• Conduct regular assessments of residents and ensure all required documentation is up-to-date.
• Ensure compliance with state, federal, and Americare regulations.
• Manage medication systems and ensure they meet regulatory guidelines and resident needs.
• Play a key role in quality assurance by maintaining community standards, assisting with inspections, and upholding a safe, home-like environment for residents.

What You Bring

• Licensure: A current state nursing license (LPN).
• Experience: A proven track record in nursing leadership, preferably in senior living or long-term care environments.
• Skills: Strong clinical knowledge, leadership skills, and the ability to manage both the administrative and hands‑on aspects of nursing care.
• Collaboration: Excellent communication and teamwork skills to work closely with staff, administrators, and families.
• Commitment: A passion for senior care, adherence to Americare’s mission, and dedication to providing a high quality of life for residents.

Physical Demands

• Stand, sit, walk, bend, and change positions frequently.
• Lift up to 25 lbs. and assist in transferring residents as needed.
• Push/pull up to 100 lbs. and maneuver wheelchairs (up to 350 lbs.).
• Assist in lowering residents to the floor during falls.
• Use safety equipment, including gait belts and mechanical lifts.
• Perform tasks requiring good vision, hearing, and verbal communication.
