Pay Range: $21-$25/hr
Job Description: At Rudolph Community & Care, we are seeking a Health and Wellness Coordinator to ensure the comprehensive physical and mental well-being of the individuals we support. This position plays a key role in overseeing medical care coordination, advocating for individual needs, maintaining accurate records, and supporting high-quality service delivery across our programs.
Key Responsibilities:
- Mission, Vision, and Values:
- Understand and champion RCC's mission, vision, and values.
- Make strategic decisions that align with RCC's goal of being the community of choice for all stakeholders.
- Medical Coordination:
- Attend medical appointments including annual physicals, psychiatric visits, and medication-related consultations.
- Coordinate transportation and staff coverage with Program Directors.
- Prepare consultation forms for appointments when unable to attend.
- Data and Record Management:
- Accurately input and manage appointments in Outlook and Therap, including follow-ups.
- Ensure timely and accurate documentation of medication administration in Therap and MAR systems.
- Create and update individual profiles in Therap; review T-Logs and follow up as necessary.
- Advocacy and Support:
- Act as a liaison and advocate during healthcare visits, addressing behavioral or medical concerns.
- Collaborate with healthcare providers to develop and maintain individualized care plans.
- Maintain strong relationships with individuals served, guardians, and other care team members
- Other Duties:
- Perform additional tasks as assigned.

Requirements:
Education and Experience:
- 1-2 years of successful Health and Wellness experience.
- Previous experience working with Disabled Adults (or Children).
- Experience with CPI - Nonviolent Crisis Intervention or Mandt.
- Extremely proficient with Microsoft Office Suite and related Program Software.
- Proficiency or familiarity with Therap is preferred but not required.
Physical Requirements:

- Ability to carry 25 pounds.
Environmental Requirements:
- Works with others on an on-going basis
- Office environment on an on-going basis
- Frequent travel within metro area.
Non-Negotiable Hiring Criteria:
- Ability to write narratives in grammatically correct sentences in English.
- Excellent verbal and written communication skills.
- Valid Driver's License with a satisfactory record.
- Maintains vehicle insurance.
- Proficient at using a variety of technology (hardware and software).
- Ability to work independently.
- Ability to work with employees and other professionals at various levels.
- Excellent time management skills.
