HealthRIGHT 360 offers residential treatment services designed to serve individuals and families with substance use disorders and co-occurring mental health disorders. Our program helps clients address addiction and related issues, increase skills, manage stressors, connect to supportive services, and build a foundation for lasting recovery. Residential programs run 24/7 and include individual and group therapy, family therapy, individualized case management, treatment planning, drug and alcohol counseling, medication support services, and crisis intervention.

The Male Community Reentry Program (MCRP) is a residential facility with a capacity of 200 beds, designed for adult males who have less than two years remaining on their prison sentences. Instead of being incarcerated in a state prison, eligible individuals can complete the remainder of their sentences in the community through MCRP. The program offers a variety of rehabilitative services focused on substance use disorder (SUD), mental health care, primary care, employment, education, housing, family reunification, and social support. Treatment for SUD and mental health issues provided by MCRP typically lasts from 6 to 12 months, ensuring comprehensive support for participants.

The Health and Wellness Coordinator plays a vital role in managing client medication, coordinating healthcare appointments and services, ensuring regulatory compliance, and providing exceptional care to our clients.

KEY RESPONSIBILITIES

Medication Management

• Record all new client medications into electronic systems.
• Coordinate pharmacy services for new medications and refills.
• Facilitate the destruction of medication when necessary and maintain accurate logs.
• Manage all aspects of medication including inventory, safe storage, handling, disposal, and logging.
• Manage medication errors and coordinate medication delivery.
• Provide education to clients regarding medication compliance.
• Create accurate monthly client medication sheets.
• Manage medical emergencies when on site.
• Handle, store, and dispose of medications and hazardous materials safely.

Client Care Coordination

• Coordinate medical, mental health, dental, MAT, and optometry appointments.
• Authorize medical, dental, optometry, and MAT services with CDCR.
• Coordinate off-site passes with CDCR personnel.
• Facilitate PCR and rapid COVID testing as needed.
• Arrange telehealth medical appointments.
• Provide case management services as necessary.
• Collaborate with clinical staff for service planning and delivery.

Documentation

• Maintain documentation in compliance with applicable standards.
• Keep accurate client care records per regulatory requirements.
• Manage collection and records of Sharps containers.

Other Duties

• Network with community stakeholders.
• Participate in meetings, training, and supervision.
• Respond to emails and updates promptly.
• Arrange work schedules per program needs.
• Complete training to enhance professional skills.

QUALIFICATIONS

Education, Certification, or Licensure

• High school diploma or equivalent required; higher education preferred.
• Certification as a Clinical Medical Assistant (CCMA) or Emergency Medical Technician (EMT).
• Valid First Aid and CPR certification or ability to obtain within 30 days.

Experience

• Experience in medication coordination or healthcare administration preferred.

Background Check and Other Requirements

• Must pass background check and livescan clearance.
