Health and Wellness Director (Healthcare)

The HWD (LVN or RN) oversees and supervises the care of all residents, providing direct resident care as needed. The role involves managing the health services department, ensuring resident care needs are met through coordination, supervision, and management of health services systems. The HWD promotes a positive physical and psychosocial environment for residents.

Responsibilities include:

1. Creating a resident-centered approach to service planning, supporting residents' rights and dignity.
2. Ensuring accurate use of level of care tools to meet residents' needs.
3. Performing assessments and developing individualized care plans to maximize residents' well-being.
4. Evaluating residents with health or psychosocial changes and adjusting care plans accordingly.
5. Collaborating on resident move-ins, considering community capacity and resident needs.
6. Coordinating with healthcare professionals and ancillary services to meet resident needs.
7. Communicating with families about health care and providing education and support.
8. Supervising and training health services team members, including delegation of nursing tasks.
9. Participating in incident investigations and implementing interventions.
10. Ensuring proper documentation and communication of resident care.
11. Managing infection control practices and staff training.
12. Overseeing health services systems and staying updated on regulations and industry trends.
13. Providing on-call support and triage for resident care questions.
14. Contributing to leadership decisions and participating in various team meetings.
15. Representing the community positively to outside agencies and at community events.
16. Working within the scope of practice, with RN oversight as needed.
17. Managing staff schedules, timekeeping, and personnel records.
18. Performing additional duties as assigned.
