Job summary

Community Healthcare Network (CHN) is seeking a Peer Educator to conduct outreach and provide risk reduction information in Brooklyn. The role includes referrals to health and support services, assisting clients with appointments, and distributing HIV/AIDS information and condoms in targeted communities.

The position supports CHN’s mission of providing inclusive, judgment-free healthcare across NYC, with a collaborative, interdisciplinary team and comprehensive benefits.

Qualifications

• Requires member of targeted communities and service groups.
• Good communication skills and experience with HIV issues.
• Good verbal and written communication abilities; knowledge of local area and languages.
• Respect for patient autonomy and ability to engage diverse populations.

Responsibilities

• Conducts outreach and door-to-door outreach in targeted areas to meet program targets.
• Participates in community health events and represents CHN at fairs.
• Provides referrals to health and support services and helps schedule clinic appointments.
• Maintains confidentiality and privacy while facilitating related services.
• Performs other related duties as assigned.
