Job summary

Gads Hill Center seeks an experienced Family Support Worker for the Head Start Partner Network in Chicago. You will develop relationships with families, support goal setting, and connect families to education, health, and community resources.

The role involves ensuring compliance with grantee milestones across education, health, mental health, and family partnerships, and monitoring data in COPA. Strong communication and organizational skills are required.

Qualifications

• Bachelor’s Degree in Social Work or related field.
• Gateways Family Specialist Credential Level 3

Responsibilities

• Conduct enrollment components including accurate COPA data entry.
• Refer families to resources and document referrals and progress.
• Help parents establish a medical home and monitor health records with the Partner Network Manager.
• Maintain COPA data on family partnership agreements, assessments, notes, and referrals.
• Monitor component data regularly and follow up on missing documentation with families.
• Attend monthly parent meetings and develop rapport with parents and the Parent Policy Committee.
• Collaborate with education and mental health staff to initiate referrals to Early Intervention and CPS.
• Perform reporting and submission of monthly partner network reports.
