The General Manager, Integrated Care Management, at Essen Health Care, leads the operational execution of Intention Healthcare's multi-program care model, ensuring continuity of care for high-needs patients.

You will oversee Care Management and RPM Supervisors, align provider pods and Integrated Care Coordinators around common expectations, and drive enrollment, documentation, billing readiness, and provider satisfaction in a fully in-person role.

Aufgaben

• Lead day-to-day operations of the integrated care model.
• Directly oversee Care Management and RPM Supervisors responsible for provider pods and coordinator teams.
• Set clear expectations for supervisor performance, including panel health and enrollment growth.
• Review supervisor dashboards and pod-level performance trends regularly.
• Coach supervisors on workload management, real-time intervention, and staff performance.
• Conduct regular performance reviews and implement corrective action plans when targets are not met.
• Own performance across patient panels, provider pods, and care management programs.
• Drive enrollment growth among eligible patients and ensure sustained engagement.
• Monitor panel penetration rates, outreach outcomes, and activation rates.
• Partner with providers and clinical leadership to increase program adoption.
• Use data to identify underperforming pods, missed enrollment opportunities, and disengaged patients.
• Ensure integrated care coordinators meet productivity, documentation, and time-threshold requirements.
• Monitor completion rates, monthly activity capture, billing readiness, and documentation quality across all programs.
• Ensure workflows comply with CMS requirements, Medicaid and Health Home standards, payer rules, and audit readiness.
• Collaborate with Revenue Cycle, Compliance, Analytics, and Product teams to improve billing capture and reduce denials.
• Identify margin pressure resulting from low productivity, missed billing opportunities, and documentation gaps.
