Job summary

Casa Speech And Development is seeking an HCBS Services Coordinator to support delivery and coordination of Home and Community-Based Services for members in Arizona. The role collaborates with members, families, DCWs, case management, and the HCBS team to ensure staffing, onboarding, service coordination, and ongoing monitoring.

Qualifications

• Experience working with individuals with disabilities, healthcare, human services, or HCBS preferred.
• Strong communication and interpersonal skills.
• Ability to work professionally with members, families, caregivers, and professionals.
• Strong organizational and time-management skills.
• Ability to manage multiple members, staffing needs, meetings, and follow-up tasks.
• Comfortable conducting meetings and communicating with families and care team members.
• Strong attention to detail and documentation skills.
• Ability to maintain confidentiality and handle sensitive member information appropriately.
• Knowledge of Arizona DDD/ALTCS HCBS services preferred.

Responsibilities

• Recruit and hire Direct Care Workers (DCWs) to support DDD members.
• Coordinate the hiring process and communicate with prospective DCWs throughout recruitment.
• Assist in identifying appropriate DCWs based on member staffing needs.
• Complete DCW onboarding in collaboration with HCBS Compliance Coordinator.
• Ensure required onboarding documentation and processes are completed prior to service delivery.
• Communicate with new DCWs regarding onboarding requirements and next steps.
• Coordinate DCW staffing needs with DDD members and families.
• Work with Patient Services and Operations Coordinator on case building and staffing coordination.
• Communicate with members and guardians regarding staffing needs and changes.
• Assist with resolving staffing gaps and coordinating appropriate DCW coverage.
• Attend PCSP meetings as appropriate and participate in discussions regarding member needs, services, staffing, and service delivery.
• Communicate relevant information from PCSP meetings to the HCBS team and follow up on action items.
• Conduct ATC Monitoring Meetings every 90 days for DDD members and document outcomes.
• Schedule ATC Monitoring Meetings with members, guardians, and applicable team members.
• Review service delivery and address questions or concerns identified during monitoring and document findings.
• Follow up on outstanding items or concerns identified during ATC Monitoring Meetings and escalate significant concerns to the HCBS Director when appropriate.
