Job summary

Old Colony Elder Services (OCES) is expanding its Community/Nutrition Program on the South Shore, including Quincy, Braintree, Weymouth, and surrounding towns. We are recruiting a HC Meals Care Manager to assess needs for home-delivered meal services and coordinate ongoing support for eligible consumers.

The role focuses on visiting homes, developing service plans, advocating for benefits, documenting care, and coordinating with community partners to ensure timely meal delivery and quality of

Qualifications

• Bachelor's degree in Social Work or related field.
• Current social work license preferred.
• Excellent verbal and written communication skills.
• Excellent interpersonal and conflict resolution skills.
• Excellent organizational skills and attention to detail.
• Strong analytical and problem-solving skills.
• Proficient with Microsoft Office Suite and other software.

Responsibilities

• Make home visits to assess needs.
• Develop appropriate service plans; coordinate and monitor services rendered
• Advocate for consumers in obtaining benefits and services, and assist as appropriate
• Maintain documentation
• Initiate and maintain communication with appropriate community resources to better coordinate delivery of consumer services as needed

The HC Meals Care Manager, as part of our Community/Nutrition Program, evaluates need for home delivered meal services by our consumers and periodically reviews and reassesses service needs.
