Job summary

COLORADO HEALTH NETWORK INC seeks a Prevention Services Coordinator to deliver person-centered, low-barrier prevention services. You will work with diverse communities on HIV prevention, STI testing, and linkage to care, while supporting education, outreach, and program quality.

The role requires flexibility, field outreach, and occasional evenings/weekends, with comprehensive benefits and a path for professional growth within CHN.

Qualifications

• High School Diploma or GED required; bachelor’s preferred.
• Strong English communication, data entry, and documentation skills.
• Experience with client databases and grant reporting is a plus.

Responsibilities

• Provides education and navigation for HIV prevention strategies, including PrEP/PEP.
• Develops culturally responsive prevention materials and outreach.
• Maintains timely documentation in CHN databases and reporting systems.
