Responsibilities

• Review prenatal/birth records and conduct parent visit assessments to determine eligibility for HANDS services.
• Conduct interviews to promote outreach and connect families with appropriate resources and referrals.
• Maintain logs, schedules, charting, and prepare reports per HANDS guidelines.
• Interpret laws and policies; provide guidance to departmental staff; monitor program compliance.
