Job summary

Purchase District Health Department in Kentucky seeks a Family Support Worker within the HANDS program. You will review clinic records and conduct Parent Visit Assessments to determine the level of community support needed for families with infants and toddlers, up to age three.

You will conduct outreach interviews, educate parents, and connect families with education and nutrition resources. Travel to family homes is expected; background checks and drug screening are required.

Qualifications

• Bachelor’s Degree in Social Work, Nursing, Early Childhood Education or related field required.
• Master’s degree may substitute for one year of experience.
• Associate degree in Nursing with licensure may substitute for education.

Responsibilities

• Conduct parent visit assessment/survey within 30 days of receiving hospital/clinic records to determine eligibility for HANDS services.
• Interview families to promote outreach and connect them with community resources.
• Develop and distribute informational materials that meet HANDS guidelines.
• Keep logs of service visits, schedules, and charting; prepare reports per HANDS policy.
• Interpret laws, regulations, policies and assist staff; monitor and evaluate program requirements.
• Review program requests and ensure compliance with federal/state regulations; may conduct home visits as needed.
