Qualifications

• Bachelor's Degree in OT from an accredited OT program required.
• Applicable state OT license required.
• BLS required per policy guidelines.

Responsibilities

• Evaluates, plans, directs, and administers Occupational Therapy programs upon referral by physician.
• Plans and conducts OT programs to restore function, prevent disability, and help patients reach independence.
• Directs and supervises activities of OT assistants, technicians, students and volunteers.
• Evaluates patients and establishes care plans including modalities, exercise programs, and patient education.
• Documents evaluations, progress notes, discharge notes, and related records.
• Coordinates patient schedules, treatment programs, discharge planning and communication with physicians.
