The HACE Health Access & Impact Partner operationalizes City of Hope’s Health Access & Impact portfolio by translating systemwide health access and Social Determinants of Health (SDOH) strategy into implementation-ready workflows, governance materials, and measurable outcomes across clinical care delivery, resource navigation, community and internal partnerships, Epic-enabled execution, and accreditation requirements. The role is responsible for building and maintaining detailed project plans that define milestones, dependencies, risks, timelines, cross-functional owners, and deliverables across the Health Access & Impact portfolio. Collects, validates, and organizes program data to support dashboards, reports, leadership updates, and continuous improvement. Success is measured by timely execution of milestones, measurable patient and operational impact, and demonstrated value to City of Hope leadership. The Partner is expected to exercise independent judgment, manage risk, and provide status updates, recommendations, and decision-ready materials to HACE leadership and cross-functional stakeholders to support prioritization, sequencing, implementation, and scale decisions across the City of Hope system.
