Job summary

Mosaic in Rockford, IL seeks a Habilitative Coordinator to bridge individual goals with real-life outcomes. You will coordinate ISP meetings, guide DSPs, and shape personalized habilitative programs ensuring high-quality care.

You’ll split time 80% on administrative duties at the Rockford office and 20% in community/home engagement, mentoring teams and maintaining strong family and guardian relationships. A on-call rotation is required.

Qualifications

• Bachelor’s degree in a relevant field is required or transcripts provided.
• Minimum of 2 to 3 years of direct experience working with individuals with IDD.
• QIDP certification preferred or ability to obtain status immediately upon hire; CPR/First Aid skills required.
• Strong critical thinking, behavioral support planning, and organizational abilities; excellent interpersonal communication for coaching teams and advocating for individuals.

Responsibilities

• Facilitate and coordinate ISP meetings; craft person-centered goals and positive behavior support plans.
• Train direct support staff in positive behavior modification techniques and program delivery.
• Observe in home settings, evaluate plan effectiveness, and ensure regulatory compliance.
• Build collaborative relationships with individuals, families, guardians, nursing staff, and leadership.
• Participate in shared on-call rotation to provide guidance during evenings and weekends.
