Mosaic seeks a visionary State Habilitative Manager in Rockford, IL to standardize habilitative practices and service planning across 27 residential locations in three Illinois regions. You will lead statewide systems, develop SOPs, and align teams to deliver high-quality, person-centered care.

Qualifications

• Bachelor's degree in psychology, special education, or a closely related human services field is required.
• Minimum of 3 years of direct experience with individuals with intellectual and developmental disabilities.
• Active QIDP status, or completion of the 40-hour QIDP training within 6 months of hire.
• 80 hours of external DSP registry training required upon hire.
• Valid driver's license and DCFS fingerprinting background checks.

Responsibilities

• Develop statewide habilitative SOPs and align services across 27 residential locations.
• Provide direct supervision and mentorship for Habilitative Coordinators across regions.
• Oversee person-centered program plans, data collection, and care coordination.
• Ensure compliance with DHS standards and Mosaic policies.
• Identify root causes from statewide quality metrics and implement proactive interventions.
• Maintain on-call regional support and collaboration with coordinators.
