Job summary

Mosaic seeks a visionary State Habilitative Manager based in Rockford, IL to standardize habilitative practices and lead person-centered service planning across 27 residential locations in three Illinois regions. You will drive statewide SOPs, mentor coordinators, and ensure regulatory compliance while unifying staff under the One Illinois vision.

With a focus on accountability and staff development, you will shape care coordination, data collection, and proactive quality improvements to deliver

Qualifications

• Bachelor’s degree in psychology, special education, or related field is required.
• Minimum 3 years of direct experience with individuals with intellectual/developmental disabilities.
• QIDP status or completion of 40-hour QIDP training within 6 months.
• 80 hours DSP registry training upon hire.

Responsibilities

• Develop statewide habilitative systems and SOPs for consistency and quality.
• Supervise Habilitative Coordinators across Rockford, Pontiac, and Macomb regions.
• Oversee person-centered program plans and data frameworks.
• Ensure compliance with DHS standards and Mosaic policies.
• Provide on-call support and regional guidance as needed.
