Job summary

BAYADA Home Health Care in Rolla, MO, seeks a Habilitation Manager (Qualified Professional) to act as internal case manager and advocate for clients in the Habilitation program. You will participate in Person-Centered Planning and coordinate with clients, families, and care teams to implement plans.

The role requires the Qualified Professional credential and at least 2 years of post-degree experience with I/DD populations, plus strong communication and interpersonal skills.

Qualifications

• Must be a Qualified Professional with at least 2 years post-degree experience in I/DD population.
• Bachelor degree required.
• Excellent communication and interpersonal skills.

Responsibilities

• Act as internal case manager and advocate for clients in the Habilitation program.
• Participate in Person-Centered Planning processes.
• Collaborate with client, family, Case Managers, Hab Techs and other teams to implement and evaluate client care needs and goals.
• Make regular client visits to evaluate progress and ensure services align with the Plan.
• Evaluate progress of goals in the Plan.
• Maintain ongoing communication with Case Managers/Care Coordinators.
• Supervise Habilitation Technicians per policy and state regulations.
• Participate in client orientations as directed by Clinical Manager.
