LOCATION: West Hills, and South Hills area of Allegheny County *

WAGE: $17.50 -$18.50/hr

WAGE
$17.50-18.50/hr

Responsibilities
The Habilitation Associate will be responsible for providing community based support to individuals based on the needs outlined in the individuals’ ISP. Duties includes, but is not limited to:
- Develop and implement goals for assigned individuals
- Prepare daily/weekly activity schedules
- Assist and instruct individuals during community based activities as per their goal plans
- Collect, maintain, analyze and interpret data on individual’s activities, as required
- Submit required paperwork within the electronic health record in an accurate and timely manner
- Provide transportation to and from activities
- Attend meetings, as required and attend 24 hours of in‑service training per in‑service year
- Performs all other duties related to the position as assigned by the supervisor
- Demonstrate knowledge regarding fire and emergency evacuation procedures and equipment
- Understand that added work or irregular hours may be required due to government agency or funding source requests for special assignments
Qualifications
- Must be at least 18 years of age or older
- High School Diploma or GED
- Valid driver’s license*
- Valid automobile insurance*
- Use of personal vehicle*
- Computer skills
- Act 34 and FBI Clearances (if applicable)
* If non-driving position applicants must have reliable, consistent access to public transport
Minimum Compensation
USD $18.50/hr.
Maximum Compensation
USD $18.50/hr.
