Responsibilities

• Prepares and maintains client charts.
• Collects medical records, personal documents, and other pertinent files for client folders.
• Monitors client's monthly financial situation including income, bills, and purchases.
• Attends doctor appointments and supports medical decisions as needed.
• Attends court hearings and participates in guardian qualification and reviews.
• Produces quarterly and annual reports to agency partners and required reporting.
