Job summary

Lutheran Social Service of Minnesota is seeking a Guardian Conservator to carry out court-appointed guardianship duties, manage financials, and safeguard the welfare of vulnerable adults within the Guardianship Options Program across Minnesota.

The role involves a caseload of 35-45 individuals, monthly client visits, and collaboration with courts, families, and a support team to ensure appropriate living environments and services.

Qualifications

• Two years of experience with similar client populations and needs.
• Must have a valid driver's license.
• Previous lived experience in making clear and unbiased thinking and decision-making skills.
• Ability to exercise sound judgement regarding best interest of client, major financial transactions, and life & death situations.
• Extreme detail attention to communications, accurate and timely documentation, and billings.
• Must be cleared by a Bureau of Criminal Affairs (BCA) background study, have a driver's license, and complete a Motor Vehicle Check (MVR).

Responsibilities

• Advocate for individuals for least restrictive life choices that enable them to live full and abundant lives.
• Work with individuals and families in the development and service delivery of supported decision-making.
• Work closely with courts, including annual reports.
• Supervise and monitor client personal and health care; authorize services as needed.
• Sign legal documents such as consents, admissions, and lease agreements.
• Work as a member of a support team to ensure appropriate living environment and services.
• Travel to visit clients at least monthly.
• Provide substitute decision-making regarding medical emergencies and other sudden changes in client's situation.
• Be responsible for the financial management and oversight of individuals assigned to them.
• Work with financial team to oversee financial assets.
• Be a member of an on-call schedule for client emergencies.

Job Summary:
