Job summary

Lutheran Social Service MN is seeking a Guardian Conservator to carry out court-appointed duties, provide financial management, and oversee care for vulnerable adults in Minnesota. The role involves managing a caseload of 35-42 individuals across a geographic region, frequent travel, and collaboration with guardianship teams and courts.

Qualifications

• Two years’ experience with similar client populations and needs.
• Valid driver’s license.
• Experience in clear and unbiased decision-making.
• Ability to exercise sound judgment regarding client interests and major financial decisions.
• Extreme attention to detail in communications, documentation, and billing.
• Must be cleared by a Bureau of Criminal Affairs (BCA) background study and have a valid driver's license; MVR required.

Responsibilities

• Advocate for individuals to enable least restrictive life choices.
• Develop and deliver supported decision-making with individuals and families.
• Coordinate with courts, including annual reports.
• Supervise client medical, personal care, and health needs; authorize services.
• Sign legal documents such as consents and leases.
• Manage and oversee financial assets for assigned individuals with the finance team.
• Travel monthly to visit clients.

LSSMN is the largest professional provider of Guardianship and Conservatorship in the state of Minnesota! As a Guardian Conservatoryou will carry out the duties of court appointed Guardian/Conservator as set forth in MN Statutes; provide financial management, and/or ensure care for vulnerable adults in the Guardianship Options Program. A Guardian Conservatorwill carry a caseload of 35-42 individuals that typically covers a geographic region of the state.

As a Guardian Conservator, you will:

• Advocate for individuals for least restrictive life choices that enable them to live full and abundant lives.
• Work with individuals and families in the development and service delivery of supported decision-making
• Work closely with courts, including annual reports.
• Supervise and monitor client personal and health care. This can include authorizing medical, therapy, treatment, support, vocational, and other services.
• Sign legal documents such as consents, admissions, and lease agreements.
• Work as a member of support team to ensure appropriate living environment and services.
• Travel to visit clients at least monthly
• Provide substitute decision-making regarding medical emergencies and other sudden changes in client's situation.
• Be responsible for the financial management and oversight of individuals assigned to them.
• Work with financial team to oversee financial assets.
• Be a member of an on-call schedule for client emergencies.

GuardianConservator Job Requirements:

• Two years’ experience with similar client populations and needs
• Valid driver’s license
• Previous lived experience in making clear and unbiased thinking and decision-making skills.
• Ability to exercise sound judgment regarding best interest of client, major financial transactions, and life & death situations.
• Extreme detail attention to communications, accurate and timely documentation, and billings.
• The successful candidate must be cleared by a Bureau of Criminal Affairs (BCA) background study and have a drivers’ license; a Motor Vehicle Check (MVR) is required.
