Qualifications

• Current LPN/RN license in the state (KY)
• At least one year of LPN/RN experience in a long-term care setting preferred
• Knowledge of MatrixCare preferred

Responsibilities

• Consults and coordinates with healthcare team members to assess, plan, implement, and evaluate patient care plans
• Prepares and administers medications and records prescribed treatments in accordance with nursing policies
• Educates patients on procedures and care plans
• Records patients' medical information and vital signs
• Monitors and adjusts equipment used on patients and interprets displays
• Initiates corrective action for adverse patient symptoms
• Performs bedside care for a wide variety of patients including pre/post-surgery
• Changes dressings, inserts catheters, and assists with IVs
• Assists physician during examination and treatment
• Responds to life-saving situations according to nursing standards and protocol
• Participates in discharge planning
• Records care information concisely and accurately
