• Must have an active Physical Therapist certification.

Responsabilités

• Provides patient/client care and assessment in accordance with standards and policies.
• Provides care per physician’s orders and plan of care.
• Takes measurements of patient status to assess care effectiveness.
• Communicates with evaluating therapist to maximize patient benefit.
• Implements education plan and identifies additional patient needs.
• Assists discharge planning and goal reassessment.
• Collaborates with patient and healthcare team to optimize care.
