Job summary

Pathfinder seeks a Group Home Manager to oversee group homes for people with disabilities in Wabash, IN. This role ensures residents are safe, happy, and able to make choices, while supporting staff who work in the homes.

You will supervise care plans, assist residents with daily activities, and collaborate with nurses, case managers, and families to keep everyone informed. A strong focus on autonomy, inclusion, and stability is essential.

Qualifications

• High school diploma or GED required.
• Prefer 2 years of experience in human services.
• Must be at least 18 years old.
• Basic language, written, and computer skills for documentation.

Responsibilities

• Ensure each person’s support plan is followed daily.
• Help people learn new skills and participate in the community.
• Keep people safe, healthy, and treated with respect.
• Assist with appointments, medications, and enjoyable activities.
• Train staff and manage staff schedules.
• Attend meetings with families and coordinate care.
• Document progress and update records accurately.
