Job summary

Opportunity Partners, Inc. in Bloomington, MN is seeking a Direct Support Professional-Exception Rate to provide person-centered support in our group homes. You will help residents with daily living skills, safety, independence, and personal growth.

The schedule includes nights and weekends; wage ranges from $17.00 to $22.00 per hour. You will perform tasks from personal care to transportation, follow medical plans, and document progress to support outcomes.

Qualifications

• High school diploma or GED required.
• Minimum of two years of direct support experience.
• Minimum of one year of mental health experience with specified conditions.
• Valid driver's license required.
• Experience with Office 365 and EMAR systems preferred.

Responsibilities

• Provide person-centered support in group homes.
• Assist with daily living skills to promote independence and growth.
• Follow medical plans and administer treatments as required.
• Provide transportation to medical appointments and accompany residents.
• Document outcomes and share updates promptly.
• Support personal safety, behavior management and crisis procedures.
• Assist with personal care tasks and mobility needs.
• Promote social development and positive interactions.
• Collaborate with the team and escalate concerns when needed.
• Maintain confidentiality and professional boundaries.
