Job summary

Center for Discovery in Sacramento, CA is seeking a Group Facilitator to lead PHP and IOP group sessions, de-escalation when needed, and communicate relevant updates to the treatment team.

The role emphasizes completing group notes immediately after sessions, maintaining accurate documentation, and adhering to policy, ethics, and Joint Commission standards.

Full-time, onsite schedule Monday–Friday 11:00am–7:00pm with a pay range of $25–$27 hourly.

Qualifications

• Bachelor's Degree preferred.
• May be working toward a Master's degree to become a therapist.
• Understand the therapeutic process and confidentiality laws; practice ethics.
• Post-offer physical examination/medical history check and TB test required.
• Valid CA driver’s license and liability insurance if driving on our property.

Responsibilities

• Collaborates with leadership to develop therapy program and treatment plans.
• Facilitates groups, workshops, and educational sessions as scheduled.
• Completes group notes immediately after each session.
• Documents services in patients' medical records.
• Adheres to policies, Joint Commission standards, and regulatory requirements.
• Assesses suicidality and potential danger to self or others.
• Delivers psychoeducation to patients as needed.
• Models appropriate eating behaviors during meals when assigned.
• May travel to other facilities for trainings.

Job description

Position Summary:

The Group Facilitator is responsible for facilitating groups at the outpatient level of care (PHP & IOP). The Group Facilitator participates in de-escalation or crisis intervention when necessary. The facilitator communicates pertinent information to the primary treatment team as needed.

Essential Job Functions:

• Collaborates with Program Director and/or Clinical Director and other professional staff to help develop comprehensive program of therapy, evaluation and treatment.


• Facilitates groups, workshops, and educational groups as directed by the program schedule.


• Responsible for completing all group notes immediately after each group.


• Document all services provided in patients' medical record.


• Knowledge of company policy and procedures, and Joint Commission standards and regulations.


• Adheres to all regulations and documentation requirements in regard to patient care and compliance of all patient care.


• Assesses suicidality and possibility for danger to self or others.


• Provides psychoeducation to patients, as needed.


• Attends meals and models appropriate eating behaviors with patients, as assigned.


• May be required to travel to other facilities to attend trainings.

Knowledge, Education, Experience:

• Bachelor's Degree, preferred


• May be working toward Masters degree to become a therapist


• Must understand the therapeutic process, understand confidentiality laws, and have strong knowledge of professional ethics.


• Successful candidate must submit to post offer, physical examination/medical history check, and TB test.


• Must have valid CA driver's license and liability insurance if driving a personal vehicle on our property.
