Job summary

Life Anew Behavioral Health seeks a Group Facilitator to support individuals and groups across clinical programs. You will lead group sessions, help develop treatment plans, and document progress in Medicaid-compliant EHR systems.

This role requires strong communication, crisis management, and a client-centered approach. Flexible hours include part-time and full-time options in Ohio, with work-from-home possibilities when appropriate.

Qualifications

• 1–2 years in case management or behavioral health.
• Proficiency with Electronic Health Record (EHR) systems.
• Experience completing Medicaid-billable progress notes.
• Crisis management and conflict resolution skills.
• Compassionate, client-centered approach to care.
• Excellent written and verbal communication.
• Knowledge of DSM-5 criteria and assessments.
• Licensed: LSW/LISW, LPC, LPCC or equivalent.
• Bachelor’s in Social Work or related field; valid driver’s license.

Responsibilities

• Provide counseling and support to individuals and groups experiencing behavioral health challenges.
• Develop and implement individualized treatment plans tailored to client needs.
• Collaborate with clients to establish treatment goals and milestones.
• Prepare progress reports and documentation for stakeholders.
• Educate individuals, families, and groups on behavioral health and prevention strategies.

• Maintain Medicaid-compliant clinical documentation.
• Complete group therapy sessions.
• Perform other duties as assigned by Clinical Supervisor or Counselor.

Job Description

The Group Facilitator will report directly to the Clinical Supervisor. Will be assigned to one of the following departments: Direct Service, Day Treatment, Therapeutic Behavioral Services, SUD Services, Group Therapy.

Responsibilities

• Provide counseling and support to individuals and groups experiencing behavioral health challenges.
• Develop and implement individualized treatment plans tailored to client needs.
• Collaborate with clients to establish treatment goals, objectives, and measurable milestones.
• Prepare accurate and timely progress reports, correspondence, and documentation for relevant stakeholders.
• Educate individuals, families, and groups on behavioral health conditions, treatment options, and prevention strategies.
• Facilitate referrals to appropriate healthcare providers or counseling professionals when necessary.
• Maintain accurate, up-to-date, and Medicaid-compliant clinical documentation.
• Complete Group therapy sessions
• Perform other duties as assigned by the Clinical Supervisor or Counselor

Qualifications

• Minimum of 1–2 years of experience in case management, clinical documentation, social work, or a related behavioral health field.
• Proficiency with Electronic Health Record (EHR) systems.
• Experience completing Medicaid-billable progress notes.
• Demonstrated skills in conflict resolution and crisis management.
• Compassionate, empathetic, and client-centered approach to care.
• Strong ability to build rapport and maintain positive relationships with clients.
• Excellent written and verbal communication skills.
• Knowledge of DSM-5 diagnostic criteria
• Must have experience doing Diagnostic Assessments and Treatment planning
• Reliable transportation, a valid driver’s license with fewer than 4 points, and proof of insurance.
• Minimum of bachelor’s in Social Work or a related field
• Minimum of a high school diploma or equivalent required.
• Must Have minimum of LSW, LISW, LPC, LPCC, LISW-S, LPCC-S
