Qualifications

• Bachelor’s degree in a human services-related field required.
• Master’s degree in counseling, social work, or similar field preferred; grief counseling certification advantageous.
• Two to three years of bereavement or grief counseling experience with terminally ill individuals.

Responsibilities

• Coordinate bereavement services for patients and families in Routt and Moffat counties.
• Conduct bereavement risk assessments at intake and after death.
• Provide bereavement follow-up for 12 months; include calls, mailings, and group support.
• Facilitate bereavement groups on a regular basis.
• Document communications and prepare quarterly reports.
• Collaborate with the inter-disciplinary team and volunteers to meet needs.
