Agape Care Group in the United States is seeking a Grief Support Coordinator to coordinate bereavement services, offer grief counseling, and assess families for complicated grief or depression. You will monitor communications and work with the care team to integrate spiritual care for patients and families.

The role includes supervising grief support activities, memorial services, and family assessments, while collaborating with interdisciplinary teams to ensure compassionate, dignified care

Qualifications

• Master's degree in counseling, theology or related field with grief/bereavement training preferred.
• 2 years of experience in health care or hospice settings; family therapy experience in loss and grief.
• Reliable transportation and ability to lift at least 25 lbs.

Responsibilities

Join Our Team as a Grief Support Coordinator Are you passionate about making a difference in the lives of others? Do you want to make a meaningful impact and help create positive experiences for patients and families? We are looking for hospice grief support coordinators who are committed to guiding each patient with dignity and compassion. In this role, you will be responsible for coordination of the hospice bereavement services offered in the assigned territory. This is accomplished by contacting families to offer bereavement support, grief counseling, and assessing families for complicated grief or depression, monitor database and correspondence to all families receiving bereavement services. Provides bereavement support through grief counseling, memorial services, family assessment and therapeutic group counseling. Responsible for consultation to interdisciplinary team, client and family in issues related to grief and loss. Working with the care team, you will develop, implement, and supervise the spiritual needs of the patient and family.

Qualifications

• A heart to serve patients and families and a passion for providing the best possible care
• Education: Master degree in counseling, theology or related field, with documented training in Grief and Bereavement preferred.
• Experience: 2 years experience in a health care or hospice setting. Experience in family therapy regarding issues of loss, grief, death and dying. Experience in developing and conducting programs of support.
• Required: Reliable transportation. Ability to sit, stand, bend, move intermittently and lift at least 25lbs and bear the weight of an average adult effectively.
